Vital records, at county office should send a form to physician signing death certificate, when cause of death is " PENDING " , or " NOT DETERMINED " , in order to finalize a case. If the physician who signed the death certificate considers that based on NEW INFORMATION, and AMENDED REPORT SHOULD BE ISSUED. He or She must request the form for " DIAGNOSED AMENDMENT " from the VITAL RECORDS DEPARTMENT, AT THE COUNTY OFFICE..........
