AMENDED DIAGNOSIS FOR A DEATH CERTIFICATE; there is a procedure, and a

form that must be filled out by the physician who signed the death

certificate. Vital records, at county office should send a form to

physician signing death certificate, when cause of death is " PENDING " ,

or " NOT DETERMINED " , in order to finalize a case. If the physician who

signed the death certificate considers that based on NEW INFORMATION,

and AMENDED REPORT SHOULD BE ISSUED. He or She must request the form for

" DIAGNOSED AMENDMENT " from the VITAL RECORDS DEPARTMENT, AT THE COUNTY
