I have been keeping it in a binder and figured I would just send the whole thing color coded tabs and all. I know this seems like a minor detail but even these are important to me. Thanks so much. OR do you send in packet, schedule a clinic, and get a surgery date on clinic day?? I have all items needed to send my packet. I know what order to submit it my question is do we send a copy of the patient information form that we filled out of the Internet? Also, do we include the letter that Dr. Rutledge sent to our insurance company? Do we include a copy of the approval letter from our insurance company? Did any post-ops send their pictures taped to a page or does he prefer you submit them in an envelope?? I am so excited I am shooting for July 12th as my day to dance with Dr. Rutledge. Thanks for all the help and support I have received on this mailing list. 98% of you do not know me, but when I needed advice or a pick me up all of you came through for me and words cannot express my utmost appreciation for that.
