Scanable SSI DISABILITY forms

Hi ya Peggie, wow, now a 'scanable form' - that's a new one to me! hmmm,

well, if this new form is so short, just give them the MOST RECENT

info. SS will contact you if they want to know 'the missing dates'/Dr.

names, etc. How wierd they don't want Dr. names. lol not your fault

there is not enough room on the form! Where it says 'reason for visit'

put 'CMT disease' or other short words. Maybe under 'remarks' you could

put in your regular Dr. name. As for the other reports, don't send

anything directly to the SS office yourself - things tend to 'get lost'!

If SS wants/needs more info, they will let you know.
