Guest guest Posted March 7, 2000 Report Share Posted March 7, 2000 Hi Peggie, yep, SSDI is cracking down now and doing more CDR's (Continuing Disability Reviews) about every 3-4 years. So, yes, fill out the forms, and yes, try to list the docs/appointments, etc. If you have to - if there isn't enough space on the pace, say 'see attachment', then list. You can do this from your word processing program then print it then attach it. Will save you some time. If you don't know dates/doctors, since 10 years in a long period of time, even if you fill in the doctor name, SS will check. And to all, I would suggest now that EVERY time you see a doctor of any kind, podiatrist, orothotists, etc, that you just write a brief summary of the visit FOR YOUR FILES. Put on there the date of the appointment, who you saw, what was said, treatment recommended, and also, how much it cost! (even if insurance picked up the tab)! This is a good way to keep your medical files up to date, then you don't have to hunt around and try to remember things, and worse yet, get the info. from your doctor ( which in my case, once or twice years ago, had completely WRONG info!) I know there has been a couple lately that mentioned getting one, but don't remember who it was. I just got my first one and have been on disability for 11 years! Am I reading this right? Do I have to list every single doctor appointment I have had for the whole (10 years) reporting period??? And do they want statements from the doctors? Thanks, any help appreciated. The real strange thing about this report they sent me, is that it has 3 lines to put the doctors you've seen and asks for Reason you went, and date - but no doctor name. For Hospitalization - 3 lines only - date and why - again, no names. It is only in the directions that it says: " Your medical bills or doctor can provide a short, accurate description. " That is because the " Reason For visit: allows 16 letters to describe your reason for the visit. Isn't that real strange? And like I said, does NOT ask name of doctor. This is a real minimal report they are asking me for. The remarks where you have extra space to use if needed is only 5 lines. This report is also a scannable form which can be " read " electronically. So, don't know if I should go ahead and add all the extras or not??? It really doesn't seem like they want any from me. It does say, in the fine print that if necessary use an extra piece of paper. The strange thing is, is the fact that they are asking so little! This is just not what I was expecting - I thought it would be the regular questionnaire that asks you everything and wants everything, and this one is asking so little that it has thrown me for a loop!!! So what do you think? And thanks for your help! I am indeed going through my calendars since 1993 (all I can find) looking for doctor appointments that I have written down! Luckily, I don't go to doctors much. Then will go through check book registers looking for payments made to doctors to double check against calendar. I really knew after first filling out all those papers that I should have kept a good accurate record of this! I do have reports (4) completed by my doctor for my private insurance company confirming that I am still disabled.... do you think I should send a copy of these reports to SS? Thanks!
