Just sent them in and I was wondering what type of chance I had. Till I hear from them I will get all the other papers to Dr. R. I'm almost done.... What a blessing! All you need to do is complete and submit the information form on Dr. R. web site. Dr. R. will automatically fax a pre-authorization insurance letter (or mail if you can't provide a fax number) to your insurance company. Even if it's out of your network, his letter is kind of over-all and your insurance would then need or ask for additional information, depending...... Meanwhile, start doing all of the other 12 steps listed to get the packet of information ready to send Dr. R. This will take a lot of time. You do not need to set up an appointment or see Dr. R until all those steps are completed. I have completed all the steps and will see Dr. R. the first time on May 16 in clinic the day before my surgery, May 17. She has an HMO plan and got a referral letter from her PCP. I have a PPO plan and was approved in about 3 days. Good Luck! I am hoping for a date the week of the May 22. My Mother and Sister have already had the surgery. Good Luck!
