Help me on my way to the other side please. Reply to ElsbethLiz@...

1. The instructions for completing the Patient Information form do not say

use third person but many of you say to do it that way. Must I? Would you

give me an example.

2. Under Detailed Description of your Diets, how specific did you get? What

is the best way to go through it?

3. On page 3, listing the diets, I'm not so sure of how long on each diet or

how many pounds I lost. How do I handle that? ALSO, there is a column

headed WEIGHT REGAINED with a box underneath it. Do I just put an X in the

box or am I supposed to put how many pounds regained? I surely don't know

all that anymore.

4. Under referring doctor, what do you put? I don't know yet that my PCP

will agree. I was going to call for an appointment to discuss it tomorrow.

So who's name do I use? Do I have to wait til I find a PCP that will support

me?

5. Did you put anything else under " Other Comments " ?

Please let me know as soon as possible any of you faithful ones out there

that will help me. Thank you so much in advance. Liz

Thanks for the information on how to complete the form online. So in other

words you are completing the form as if you are Dr. R. as if he were asking

the question and competing the form himself.

The online history form is what Dr R uses to fill out the insurance

application, and also the appeal if you are denied. Thus you need to fill

out the form as if it were Dr R talking about YOU. So instead of, " I've

tried several diets in the past, " You type, " Mrs. has tried several

diets in the past. " Use she or your name instead of I. Hope this helps.

Otherwise the insurance forms and your appeal looks really funny.....it

looks like Dr R is the one who weighs 300lbs. LOL Otherwise, you will have

to do as I did.....ask Debbie who is already overworked to edit my papers so

it looks right. :-)
