Yes, I believe they want to know if you have an HMO, PPO, POS, etc. If you

must contact a Primary Care Physician (PCP) first before getting a referral

to Dr R, you have an HMO. Otherwise it's prolly one of the others, which

you can find that out by looking in your insurance booklet or calling your

HR rep at work, or the insurance company itself.
