Is PCP visit your first step?

Hi..just have a question..I was looking at the online form to fill out and I

see that it asks for your physicians information so am I correct to assume

that the first step in this process would be getting a physician to say he

will follow you through? Then step 2 would be filling out the form? I am

moving back to Illinois (which is home) in 4 weeks so I guess I can't start

my process til then til I can talk to my doctor. Also, how do I know what

tests need to be done? Is that in the manual? Thanks for your help again.

I read all the posts everyday and am following the progress of everyone.
