Liz I'm going to try to help. Go to the clos.net site, on the left side of

the screen where it says,Start Here, click on that, the page showing the 12

steps to go thru should come up, go down to where it says something about a

patient letter and you'll see where it says sample patient letter with lines

around it, click on this, it should bring up the sample letter, then if you

want to print it just click on print at the top of your Internet home page.
