Do we have to print off the ins letters (from his web site), and tweek them to fit our circumstances and submit them to our ins by ourselves? Confused and anxious to get underway with the hmo. Guest guest Posted May 1, 2000 Report Share Posted May 1, 2000 It's true that Dr. R will submit your insurance request, but remember to PRINT OUT THE PATIENT INFORMATION FORM BEFORE YOU SEND IT! Once you submit it, you can't get it back.
