Guest guest Posted May 17, 2000 Report Share Posted May 17, 2000 Dear Genz, Thanks for that invaluable tip on sending in the Patient Information form! I had two more mishaps, a couple of anguished calls to Barbara and Debbie, and then began opening my email like a crazy person after answering every single question! I also began bookmarking the form after every three questions. (I have some cleaning up to do of my files after this.) The final straw was when, after I had completed everything, the program kept kicking back the form, saying I needed 11 numbers in the SS # field!!! I called back to Dr. R.'s office, and Barbara allowed me to send in the form with a made up SS #. And it went in! Finally! So what happens next? Will I hear from my insurance company any time soon? I'm meeting with my PCP tomorrow, had my mammogram, seeing the gynecologist next week, and starting to write my letters. I hope this all comes together more easily than the PI form.
