Do We Have to Have Support?

This may seem like a dumb question, BUT I have read the and it sounds

to me like we have to have our PCP support before we have surgery and they

agree to take care of us after the surgery.

Then another time I read it as long as we have the PCP agree to take care of

us after surgery that is all we need from them.

Which is it?
