Do any of you know what the difference is between "en bloc" removal and "capsulectomy?" Which is better? So the surgeon cuts around the scartissue and removes it whole.....that way there is no spillage of debris that is between the implant and scar capsule....and if by chance the implant is ruptured, you dont have contents of that flow throughout your chest wall. Implant removal and capsulectomy is where they open the capsule and remove the implant, then they cut out the capsule....huge difference. see the attached pix of implant that is showing gel bleed due to implants being porous. You dont really want what ever has leaked or grow in there to be leaked inside of you..... That is the removal of both the implants and the scar tissue that has formed around each implant.En bloc would be better in the cases of ruptured silicone gel or saline implants. It would probably be better for everyone, but it is not always necessary. However, a total capsulectomy is almost always necessary.Here is the difference:En bloc involves NOT cutting into the scar tissue (also called "capsule"). The surgeon uses his knife to dissect around the perimeter of the implant and scar tissue to remove it from the breast. Picture an M & M Peanut candy with a peanut and candy coating. The peanut would be the implant, and the candy coating the scar capsule. This would be removed whole.In a total capsulectomy, the surgeon may cut INTO the scar capsule, and remove the implant first, leaving the capsule behind. He will then have to remove the capsule separately, by possibly cutting the capsule into pieces (the anterior, or front piece, and the posterior, or back piece.) Picture an M & M peanut candy in which the coating is cut into and the peanut taken out, leaving the candy coating shell. The reason a TOTAL capsulectomy is necessary is because it means that the total and complete capsule is removed, not leaving any pieces in the body.I hope that has helped form a proper image in your mind of what is being done when implants are removed. Either procedure is sufficient in the case of unruptured implants, IF in fact, the capsulectomy is a TOTAL capsulectomy.
