I do believe that it's easier and more accurate to keep track of everything when it is written out clearly. As you mentioned, also, it's easier to remember questions for the doctor, when you jot them down as they arise. Thanks for letting us know about your using the charts. It would be nice to see what other people are using, also. There's always room for improvement, and everything seems to change regularly, too.
