Sandy,

I agree, please don't spend your money on snail mail thank you

cards. I did receive your email and that's more than enough I

think you have all the paperwork you need. If they don't approve your

surgery with the info you have then someone's got their head attached

incorrectly and may need a surgical procedure to remove it!
