You do not have to fill out the questionaire online or fax it in, etc. - You can call the 800 customer service number and inform them. They will take some basic information like name, address, etc. and then they will give you a fax number that your surgeon can send the script to (if they don't ask them). My surgeon's nurse just faxed the script to them and the order was put through pretty quickly.
