Hi Terri--

I don't know if it would be official enough for your health insurer, but why

not print the list of surgeons who do DS off the DS.com Web site at

http://www.duodenalswitch.com/Surgeons/surgeons.html? You could add a short

statement about the webmaster who maintains the site and the way they keep

the list current. Then I'd provide it to the dr. who needs to write you the

letter and if s/he won't cooperate, then send it in yourself in your appeal

package. Worth a shot?
