I would mention that nothing that someone with a PPO policy has been approved for applies to an HMO policy. And in spite of the fact you seem to feel that they should have given me and others a written approval, the PPO policies simply don't do it. You have a very slim chance of getting approved with a UHC HMO policy. I'd recommend your contacting an attorney, Walter Lindstrom at www.obesitylaw.com comes to mind. I wouldn't use up all my denials before contacting him as you definitely will get nothing but denials. He might be able to help and would tell you up front if he can't. Hope you find some help.
