Out of Network Approval/Referral

Hi,

Here's what hopefully will be a simple question.. What do you have to

get a referral to an out of network doctor if you have a PPO ? More

specifically, does each doctor have a specific referral ID# or

something that is also non-insurance specific like insurance codes(ie

43846, 43838, etc) so it's easier to request a referral to an out of

network doctor ?

I asked already on OSSG-Insurance, but I didn't get an answer, so I

was hoping you guys would know!
