interpretation competency

We are developing an interpreter competency program here in the hospital system

where I work. We have completed a draft of a form we'd like to begin using. I

would like the expertise of this group to critique what we have done so far.

Does this form seem reasonable? Where could it be improved? Does this form do an

adequate job of assessing someone's written and oral skills as an interpreter?
