Ann Marie,

I thought I had saved the document I had attached, but of course I can't

find it. I told them that cramping and spasms made it impossible to sit

for long periods of time without getting up and moving around. I also

told them that because of bladder weakness, I may need to visit the

facilities more often than the regular brakes given to a jury. I wrote

it up and sent it to my neuro and he signed it and sent it back to me

right away so that I could attach it to the questionnaire. Either that,

or the questionnaire said something about if you put down reasons on the

questionnaire, then you needed doctors confirmation within a certain

number of days. My had my doc indicate the I will get worse in the

future, not better and I haven't been called since. I hope this helps a

little bit.

Forms may vary from state to state, but my doctor always helps me out when I

get called for jury duty. I give them his phone number and he verifies that

I cannot serve. For a while I seemed to get a notice for jury duty once a

year, and I inquired if there was any way they could permanently take me off

the jury duty list. They said no, my name will remain on the list but I will

not have to serve. They've always been helpful and never given me a hassle.

I just received a Juror Qualification Questionnarie. I have always wanted to

serve on a Jury and feel it is my duty as a US citizen.

I am wondering if anyone has gotten a Juror Questionnaire and what they

me, and I know if I write that I sleep for 18 to 20 hours a day they will

think I am just a LAZY person.

So if anyone has any idea how to explain to the courts that Mitochondrial

disease is a physical disability in short-form please respond. Any and all

ideas are welcome.

I finally got called for jury duty many years ago, something that I had also

wanted to do when younger. When I asked my doctor if he thought it would be

ok he said no it woulnd't. He had me taken off the list permanently. I

received a notice from the state saying that I was off the list and would

never be called. I never had to do anything. The doctor just did it.

I think state laws must vary. They told me that legally they could not

remove me from the list even though I would never be able to serve. Glad

your state allows for this. It makes sense.

Hi Barbara,

According to my questionnaire, they will not call the doctor to confirm that I

have a disability and can't serve on a jury. I guess it does differ from state

to state.

I can mail a letter from my doctor along with the questionnaire but they won't

call the doctor.

I have an appointment with my Pain Doctor tomorrow, I wonder if they would

accept his statement that I can't serve on the jury, or if they would rather

have a Neuro, or Mito doc write and sign the letter.

I lucked out on jury duty. I was called after being a registered voter for

more years than many of you have been alive. I called after getting my

letter and asked if the court-room was accessible. They said that they could

get me in through another court-room and the judges chambers. Then I asked

about the waiting room for the jurors - the door was too narrow. I guess

this means, hope for an old court-house.
