There's no requirement that I know of to follow a set protocol. Quality of care varies, just like with any subset of medical practitioners. The older methods of chelation (sometimes with a different med, by IV) had more potential for distressing side effects that the current, slow method with Chemet (OD almost impossible) and ALA (amino acid) and supplements to support the process and replenish any lost minerals. The Autism Research Institute is in the process of completing a new protocol, which has many safeguards and preliminary tests. My personal experience is limited, but the only side effects experienced (or reported by friends) have been very minor - sniffles, a rare diarrhea occurence, a little spaciness. Since my son was almost 10 when we started, I dream of complete recovery, but don't bet on it.
