Guest guest Posted September 27, 2005 Report Share Posted September 27, 2005 Sometimes there is a box for ALL but if not, check all written, lab, xray & other and also write in that you are requesting copies of ALL records contained within your patient record, including all labwork and xray, surgical or other diagnostic procedures and treatments. Then, when you get them, check for any obviously missing data. (Like a set of MRI films that are numbered 1-12 but slides 5 & 7 are missing).
