Need Advice About Medical Records

I have decided to get a copy of my medical records to have here for my own

information and reference.

I have requested 'request forms' from my gyn/onc, my gastro spec, and the

hospital for my two surgeries.

Now, I remember there being lots of little places to mark exactly what records

you want.

Well, I want them all, probably including some they don't have listed.

How do I do this? Should I make notation that I want copies of everything they

have in my file?

I will know more once I get the forms and may come back asking for help from you

all in filling them out:)
