I don't know exactly how the dead stuff gets cleared out, I imagine it is done by monocytes or maybe other white blood cells? And by phlegm - well, I don't have much, but there is some of that. In any case, I think the most important thing is to minimize the symptoms by keeping dose to a tolerable level, just like chelation. This gives your body a chance to deal with the the die-off so it doesn't get overwhelmed. Liver support might be helpful for some people during this process.
