If you do not, it would cause, say a sinus rhythm at 80 with bigeminy pvcs to be calculated at a rate of 40 if you did not count the pvcs, which would be incorrect... PVCs will cause the ventricles to contract, which will generate a pulse under normal circumstances... it will not necessarily be a strong pulse to to the lack of atrial kick, but generally, a pulse nonetheless.
