alert to person place time and event he answers your questions

appropriately however you can sense anxiousness in his voice he tell you he was

watching when tv his arm began to feel " heavy " and he felt a " fluttering " in

family his pulse is palpated and is strong yet he is tachycardic at 101.

Blood pressure is slightly elevated at 140/90 which he tells you is high for

was repaired with disc replacement surgery, and he went to the ER several

weeks ago for " almost passing out " in the grocery store. He has taken adderol

and prevacid in the past. He describes this feeling as an anxious feeling

in his chest and he gets " extremely tired&qu ot; after this episode. When

place in the ECG he is sinus tach at 101 with no abnormalities per protocol

you run a 12 lead which is unremarkable.

How about heart sounds? Has the patient ever seen a cardiologist?

Heart sounds are normal Pt. tells you he has never seen a cardiologist for

this problem but he has had a stress test done in the past year for a work

physical and it all was normal. He also had full cardiac workup " afew weeks

ago " when he went to the ER and all was normal. He sees his PCP once every

few months but has not followed up with him since his last ER visit.

Oral temp is 98.8. BGL is 140 pt tells you he ate dinner about 45 mins ago what

else you want
