sometimes I think that part of sleep reversal also has to do with being in the hospital environment itself.

I am guilty for part of this for some people. I work as a C.N.A. on a cardiovascular/ step-down unit. And I know that we (C.N.A.s, Nurses, Lab Techs, etc.) come in at odd hours and end up waking people at all hours of the night. I know that I have to do vitals at midnight and 4 am, plus we are required to get our patients that need to be weighed (cardiac pts, liver failure pts) up and weighed between 5 and 6 am because the drs want to have the info when they come in.

Plus another thing I think leads to it is that when you are in a hospital for a prolonged time you do not follow your normal routine. So you body sleep and wake cycles get messed up. I have also heard of something called ICU psychosis-which is when people are so confused when they come out of ICU they almost don't know up from down, and their body cycles are all messed up because stuff in ICUs is done to pts 24/7.
