Wow – I didn’t know that waking up in the middle of the night

and not being able to get back to sleep was a symptom! Thanks ! Yes, I do

that sometimes.

I found this stuff through my sister...Tension RX nighttime...by Biochem....it's pure

#5 ...hydroxytryptophan...You can by it on line or at a heath store.... You don't wake up groggy..Hang in there

I had almost exactly the same problems you descriped pre-transplant, and

there is some research that suggests that it could have to do with

altered melatonin levels in liver disease. For me, taking melatonin

supplements really tended to work well. The only thing to be aware of

is that since it's processed by the liver you want a pretty low dose.

I think I was taking about 1mg or less. I also found that it worked

best to get me back on schedule, rather than an every night thing. Once

I was back on a good sleeping schedule, I'd stop taking it and I'd be ok

for a while (days or weeks). When needed I could start it up again.

Not sure if you can get melatonin in the UK... I seem to remember some

of our Canadian members having trouble getting it.

Unfortunately it's not available except by prescription and most medics

over here don't like to prescribe it! Maybe I'll get in touch with my

transplant team and see if they have any advice.

Fortunately I'm still at home and even occasionally getting in to work

so I guess compared to many people I'm relatively lucky. Best thing of

course will just be to get the call soon!

I have no experience with 5-HTP (Hydroxytryptophan), but I'd be leery

of trying it without medical advice.

http://www.cochrane.org/reviews/en/ab003198.html (meta study) states:

The researchers reported that the symptoms of depression decreased

when 5-HTP and tryptophan were compared to a placebo (non-drug).

However, side effects had occurred (dizziness, nausea and diarrhoea).

They also reported that tryptophan has been associated with the

development of a fatal condition. More evidence is needed to assess

efficacy and safety, before any strong and meaningful conclusions can

be made. Until then, the reviewers propose that the use of

antidepressants which have no known life threatening side effects

remain more attractive. The review sets out the required methodology

for effectively studying these substances in proper controlled

studies.

Purity doesn't mean anything if the substance is ineffective or

harmful.

I'm a post tx patient, but remember the sleep reversal/insomnia pattern well! I will say that the Rifaximin (Xifaxin) made a huge difference for me where the encephalopathy was concerned. I did not have the 'flapping' or trouble drawing, but certainly had the confusion and trouble locating the right word(s) when speaking. Without the Xifaxin I could not have functioned well enough through the 2.5 years I waited for a tx - it was difficult with it.

Hope you can discuss this medication further with your doctor and find some relief soon.
