The take a set of pictures first, then administer the contrast and then take another set of picutres. I am allergic to the iodine in Gadolinium so I don't get it very often, and when I have to have it, I have to take a lot of medication before the test, so I really can't comment on the pain issue. As for the beading, from what I understand that just indicates an area that is begining to be damaged fromt the PSC. Mine started in the left lobe and then spread throughout. Guest guest Posted March 16, 2009 Report Share Posted March 16, 2009 I'm fairly certain that the string of beads appearance can be just in the left hepatic duct since PSC does not necessarily affect the entire liver evenly. That is why a liver biopsy may miss the PSC diagnosis. I have small-duct PSC, so only the small ducts are affected, at least initially. Small-duct PSC often becomes large-duct PSC.I'm not sure if you have heard about the tree analogy for the bile ducts. The main bile duct would be the tree trunk, and the smaller ducts are the branches of the tree, until you get to the really small tree twigs (those are the small ducts, often at the outer edges of the liver). The tree may become sick in one area, but not in other areas. However, the PSC sickness is progressive and tends to spread throughout the biliary "tree" over time.I can't answer about the gadolinium. Guest guest Posted March 18, 2009 Report Share Posted March 18, 2009 When I was diagnosed with PSC via ERCP in 1989, one side showed the typical beaded appearance of the bile duct and the other was smooth and unaffected. The doctor said that PSC does not necessarily progress evenly through the liver.
