After he fills out his part do I need to go in and type what he wrote and then submit it via the internet? I just want to make sure I do this right. Anybody please help! My doc just wrote a letter with my medical problems, a statement that I was medically fit to undergo surgery and he attached my blood test and ekg. He also stated that he was willing to follow me medically after surgery. That must have been fine with Dr. R because here I am on the other side! My PCP did my physical and sent a copy of his own paperwork and what I filled out prior to my physical. I did give my PCP the example form to use as reference but I told him he did not need to fill it in unless he wanted to. He chose not to fill it out and I didn't have any problems. Just FYI.
