Guest guest
Dee, When we compiled our packet (5/14/00), we put it in a three-hole binder with colored tabs. We also had contacted everyone by email, in fact we had about 90 responses and we included them all in the packet. At clinic 2 days before surgery, when we turned in our packet, they said it was great! However, the presentation is not as important as having all of the required documents. Good Luck with everything! Patti in MD
