For you patient contacts, he just needs the name of the person and a summary

of want you talked about (e. g.,),, diet, recovery, etc.

For the letter of understanding he wants you to address each topic that he

outlines on page 20 (?) of the manual. He appreciates the pats on the

back...but your letter should address your understanding of the procedure.

And don't forget to sign it.

The letter from your physician should say you are mentally a physically able

to undergo the surgery and that he commits to your follow up care. I think I

may have the letter my doctor wrote. I will email you. Hope this helps.
