hi, my insurance company also requested a pshc. eval. all they really want

to know is are you mentally capable of decideing whether or not to subject

yourself to elective surgery. and do you know what you are getting yourself

into.. its usually a one session thing, go in and tell them right at the

beginning that you are there because your ins. company is requesting it for

pre-eval for elective surgery and they will know what to ask you then. and

just think you have to have it for Dr. R anyway so that will be one less

thing you have to get done prior to surgery.
