They told me over the phone that yes they do approve this surgery but now they are wanting a physical history. Does Dr. R's office take care of this or do I? I don't want to leave this hanging for too long else they lose my paperwork in the trash or shredder! Guest guest Posted June 21, 2000 Report Share Posted June 21, 2000 , Nope, don't rely on the office. They submit a letter ... that's it. Anything supplemental is gathered by you. Gather these things and send them all at one time. I will give you direct address to Medical Review. You need to send these things, otherwise, they will deny you and tell you what they need one at time: A detailed diet history (They want to see at least one documented physical diet), your medical records from your PCP should detail your weight and any comorbidities...they tell some people they need to be double their ideal weight), A letter from you stating why you need the surgery (real sappy...I will send you mine for ideas.), A letter from your PCP stating medical necessity (optional...but it helps). They may want to attend clinic first. They did not pay for my visit, but I didn't matter to me. I will email you personally the address to send this to.
