can kick all the butts on the list, Sorry Robynn she might get you..lol

Actually she is a really sweet young Lady. Well, not to young..hehe

She has 2 little angels. Don't let her get away with anything, she can

handle it..lol She is having the other surgery here. I guess I will have to

take care of her. She is worth it..Hey le, She looks a lot like you.

A real cutie. She really needs our support, she is doing this on her own.

Hi Tonya!

Welcome aboard! If you are just starting on the process of applying

for the MGB, first thing to do is go to http://www.clos.net and fill

out the online patient questionnaire. That is what Dr. R. uses to

send to your insurance company and it gets the ball rolling.

Be sure to print off two copies of the online patient questionnaire,

too, because you will have to have one when you go to your first

clinic (or otherwise you will have to fill it out again at the

clinic)

and you will want one for yourself.

While you are waiting on that, you can be getting all the other

things

you need together in one packet (read about it in the patient manual,

also online at http://www.clos.net):

1. Your own written patient letter (it has to be 5-10 pages long and

cover all the risks and benefits of the surgery. You will find all

this in the patient manual).

2. Your family doctor's support letter

3. A support letter from your family

4. Two good pictures of you (dressed), one full-length from the side,

and another full-length from the front.

5. Your last few years' medical records (only stuff that is

significant, not routine colds, things like that).

6. (These have to be done within 30 days of your surgery, so wait

until after you get insurance approval and have a surgery date and

get

these):

Physical exam, mammogram, PAP test

Bloodwork: CBC & electrolytes

EKG

Your insurance may require that you prove you have had a year

or more of physician supervised weight loss efforts in the past. If

you have seen your family doctor about your weight in the past, s/he

may be willing to write you a letter saying " I have been the

phsyician

for Tonya for the past __ years, during which time she has been

unsuccessful in many weight loss programs, including weight loss

using

(Meridia, phen-fen, etc.) "

Your insurance may also require you to have a psychological

examination to determine if you are competent to decide on the

surgery. You may be able to get around this by having your family

physician write something to the effect, " I have counseled Tonya on

the risks and benefits of weight loss surgery and I am satisfied

that she is mentally competent to make this decision and to

follow the post-surgery requirements. " Otherwise, you can make an

appointment with a psychiatrist and get an evaluation.

Get 1 or 2 cookbooks www.lucyskitchenshop.com

Start with the intro diet, then stages.

Later www.digestivewellness.com is a must.

There are many helpful experts on this list.

My daughter has been on this diet since the beginning of January. She

had a severe reaction to her last DPTP shot (18 mont and started with

a seizure disorder and after 2 years she started to loose control over

all her fine and gross motor skills and speach. for the past 10 years

we have tried many things starting seizure medication and antibiotics

and than moved on to the holistic approach. We found that homeopathic

remedies work very well for her (not control the seizures, but colds

etc.) Question I have can she still take the homeopathic globulies?

Being on this diet, I have noticed that she gets constipated when I

give her to much almond flour. She sometimes had a problem with

constipation. How much almond flour is good for you a day. I am trying

to put weight on her. She is 14 years old and weights 52 pounds.

Your answer is appreciated .

If the globules contain sugar, then no, if the homeopathic

prepartions only have alcohol as the other ingredient then they are

okay.
