He doesn't necessarily need your story,doesn't need to here how much you need the surgery. He wants to see that you understand how this surgery differs from others,that you speak to each risk involved,and that you understand the life long commitment. It usually ends up 5-6 pages long. I listed the potential risk factors and what I could do to decrease those risks ,etc. I also outlined,briefly, my family dynamics and how I thought this surgery would effect them,etc. Good luck in your journey,hope this helps.
