patient form

I have a question (as always). If I am still working on getting insurance, do I

need to go ahead and fill out the online patient form located on the website and

just leave the insurance part blank or wait until I get insurance? I have

printed out the and I am working on my letters and I will make an appt

with my PCP next week. I feel sure he will support me.

I WILL BE calling Tuesday to check on BCBS just in case Great West does not

cover the surgery. I do see it listed in the insurance companies page

http://clos.net/insurance_company.htm but I don't know if that means they just

list it as a company you can try or if they have actually used them before and

they paid. Does anyone know?
