Guest guest Posted July 3, 2000 Report Share Posted July 3, 2000 Hi everyone, I am putting together my packet to send in. At one time I had found the forms to bring in to have your PCP fill out. I just tried to find them again and all I can find is the Sample History and physical exam and the History & Physical form which looks like the same info I filled out and sent in for the Pt information at the beginning, not anything my Dr would fill out.
