When you exhale through the flutter your breath moves the steel ball inside. This causes airway vibrations that loosen mucus, and a slight increase in pressure that aids in keeping your airways open. The combined action of vibration and pressure results in mucas moving progressively up your airways until you can easily cough it out. I know Amber used the vest while she was in the hospital last time and it seemed to really help loosen the mucas, then she would use the flutter.
