This is the hardest for most people. Take a copy of what is needed from your doctor from Dr. R on-line site with you on your appointment. When you leave if your doctor agree to support you get his/her fax number you will need it later. Call your Ins Comp and get there fax # for claims approved. When this is all done go to Dr. R site on-line and complete the on line form. Also complete the appointment form I completed this form after I had my physical, and Pap Smear I'm not sure if this was to early. Or not I'm just starting also but following the guide of others.
