LOL, Thank you. I emailed these exclusions (what I saw in the State Farm

policy) :

(1. Cosmetic surgery , unless caused by injury or by a congenital anomaly in a

newborn child who is a covered person at time of birth.

and

2. Any sickness or injury for which symptoms existed or medical advice or

treatment was recommended or received within a five year period preceding the

coverage effective date unless fully disclosed on the application and not

excluded by endorsement.)

to Debbie and ask what she thought and her response was

" Neither one of these addresses weight reduction. You may want to call State

Farm and use procedure code 43846 "

So I am going to try that. I will keep you posted.
