Also, I've been thinking. Why does every product contain comedogenic ingredients -- It's as if there is a conspiracy to clear you today, but keep the comedones forming so you're hooked on the product for life. Trust no one. One of the reasons Accutane is so sucessful, as a treatment for both acne and rosacea, is that, historically, when a patient starts accutane, they stop all topicals! Comedones are expelled, and do not reform, in part becuse of the action of Accutane, and in part because the comedogenic and IRRITATING TOPICALS ARE REMOVED. Further, since Accutane expels microcomedones that one would otherwise not see for many weeks in the future, and good test for comedogenicity (hey, forget rabbit ears) is to test on Accutane patients post clearing. A product which is comedogenic will be readily apparent within 2 weeks on the skin treated by Accutane. This line of reason just backs up my algorithm testing phase of the topicals. I noticed a post about Dr. Bitter wanting all his patients on minocyline in preference to doxycyline. Now, who else advocated minocyline on this board -- hmm could it be... me? Let us all agree that minocyline is superior to doxycyline. With regards to age and rosacea, being young doesn't exempt one from rosacea anymore than just being old doesn't exempt them from acne. And consider this, being young, getting a lot of sun exposure, a person in their 20s starts developing rosacea. This person has already gone through the general teenage acne and recovered. However this hypothetical patient then starts apply myriads of rosacea topical treatments and moisturizers, etc. This leads to a cosmetic acne syndrome on top of the rosacea, worsening everything. Or, the following scenario. A person is out in the sun chronically. We know sun is comedogenic and develops acne weeks later. This person applies irritating acne topicals. This person develops rosacea on top of the acne. Either scenario is quite common. The two nefarious factors here are: SUN COMEDOGENIC OR IRRITATING TOPICALS*. We must strive for the elimination of both of the above. Whatever the cost in lifestyle. *And remember, there is a presumption of comedogenicity of a topical unless you have tested it for two weeks bid WHILE ON ACCUTANE, post clearing.
