[MiniGastricBypass]when to send packet

I did my patient info form, and Dr. R's letter is at the insurance company

(whether they admit it or not!) I have been waiting to send in all the rest of

my

stuff because I thought that you had to have the insurance approval in the

package

with all the other stuff, but from what I have read on this list it seems like

some people send the whole package in to Dr. R before the insurance approval

happens. Should I go ahead and get my package ready and send it in, even if I

don't have the insurance approval yet?
