I just sent out my letters for patient contacts and also anyone here who would like to help me out with those...I would be forever grateful. Please let me know if this is right. Can I go ahead and do all of this before filling out the patient info online? Patient manual is already printed out and starting to look 100 years old from all the reading and rereading :-) I am waiting to fill out my patient info (online) because I do not have insurance " yet " !! (I called BCBS of Miss today and they are mailing me info on the policy and I am still waiting on policy info at Great West. I emailed Dr R's office a couple of weeks ago and asked if they have ever dealt with GW but no answer yet so I am waiting patiently) Can I go ahead and see my PCP for my assessment and get his support letter? (He gives me the letter to send in with the packet?) Get my history and physical done? Get a copy of my significant past medical records? (All I have are 2 vaginal births and 1 c-section.) Do I need copies of all my yearly paps? Just the last few years or not at all? Awaiting emails from prospective contacts. Family support letter? Patient letter? Dangers of alcohol? Photographs (front and side full body and facial photographs; name, present address, contact telephone number and body weight at the time of the photograph wrote on the back?). Then ......arrive on a Monday for clinic on a Tues. Surgery on a Wed, back to hotel on Thurs (I hope) and going home on Tues? Do I have this all correct? Please let me know if I forget anything or added to much. Thanks everyone. I have put this off long enough for fear of being turned down but I have talked with my family and done some soul searching and I know its time to do what I have to and what I can and put the rest in Gods hands.
