Is anyone out there willing to share their protocol? What questions do you ask? Any specific forms used? Who conducts the questions? HOw do you update this information? How often? Any information will be useful. I would like to share this information with the CHC participating in the Health Disparities Collaboratives so that we can more accurately track trends in migrant statistics. If all year, how many years have you lived here?, Do you work in agriculture or a factory? We have them fill out a paper including all family members and give each family member a laminated card with our name, doctors, phone # and labeled migrant 2004. This has to be repeated each year to qualify for our program.
