Photoderm / v-beam / laser / ets responses: new site

Hi,

I have created a simple online database and a form which people can

use to record their responses to Photoderm, laser, etc or other

similar treatments.

Please feel free to add your report and if you have any comments

about the form or questions asked, email me privately. Over the next

couple of weeks I will update the site with a more friendly interface

and based on any comments, but the bare bones are there which is the

most important thing.

Hopefully we can build it into a really useful database of how we

respond to these treatments.

If time allows I will try to re-create the questionnaire that someone

tried some months back, with an online form and real-time reporting.
