Lists what meds we take, dose & frequency Who, where, what our docs are What surgeries we've had, dates Drug allergies That's page 1 Then 2 docs in a row asked for what supplements we take. That'd be page 2-10 for me. LOL! Actually, page 2 is the non-Rx whatevers we take, period. After all these years, finally written in ONE spot. Page 1 is my name, address, phone #, insurance information, and my Primary Care Physician with address and phone #, medical alert information, medications and supplements I take plus the surgeries I have had along with surgeon's names and contact info. Page 2 is a list of current ailments, how ailments are being treated, and a list of surgeries and other hospitalizations. Page 3 is emergency contact information. I have a list of 3 family members, 3 friends (local), my home health aide and the health service that provides my home health aide. Since a lot of this information is asked when you see a new doctor, go to the hospital, etc., I just put it all in one place. One copy is on my refrigerator. One copy is in my medication box (the stuff I use all of the time). One copy is in my purse. And my home health aide has a copy. There was a concern a year and a half ago when I had congestive heart failure that I might have to be rushed to the hospital.
