Everyone,

I would like to invite all that are interested to sign up to become

exhibitors at the upcoming 14th Annual Midwest Farmworker Stream Forum

to be held November

18-20, 2004

at the Adam’s Mark Hotel

in Denver, Colorado.

The Forum attracts more than 300 administrators, clinicians, social

service

providers, outreach workers and staff from migrant and community health

centers,

advocacy organizations, and state/local health departments which serve

farmworkers in the Midwest

states. Hosted annually by the National Center for Farmworker Health

(NCFH),

and sponsored by the Human Resources Services Administration (HRSA) and

the

Centers for Disease Control and Prevention (CDC), the Forum offers a

variety of

migrant-specific, health-oriented educational seminars for the migrant

health

community.

An

integral part of the conference is the

Exhibitor Area which, in addition to valuable organization and resource

information, also traditionally houses photographic exhibits, silent

auction

materials and other similar attractions. All snack breaks will be

displayed in

the Exhibitor Area in addition to a special mid-day reception.

Exhibitors are scheduled to setup November 18th between 2:00pm-6:00pm. Exhibits will

be open Friday, November 19 and Saturday, November 20. As an exhibitor,

you

will be provided with a 6-foot table and two chairs. Enclosed is an

exhibitor application and in order to confirm space

and include your organization's name in the Conference Program, please

return

the Exhibitor

Application with fees by October 18,

2004.

Tables will be reserved in the order applications are received, so do

not

delay. If you have any questions, please feel free to contact me. We

look forward to your participation at the Forum.
