IMPORTANT REMINDER!!!

Farmworker Health Services, Inc. (FHSI)

recently sent a mail survey to all Migrant/Community Health Centers and Migrant

Voucher Programs. This mail survey is

part of FHSI’s biannual national needs

assessment of farmworker health care organizations. In November 2004, all M/CHCs

and Voucher Programs will receive a free copy of FHSI’s

needs assessment report.

As a reminder, the survey is a small

brown booklet with a stamped return envelope.

Please complete the survey and mail it back to us by May 15th (we must

receive it by May 19th).
